Note: If you are requesting a trial class for another student please submit a separate trial form. *For the trial, we have a limited amount of jazz, ballet and tap shoes that your student can borrow or they may go barefoot. If you don't have the proper garments yet, please just have them wear leggings/shorts/capris and a fitted tee or tank.

Medical Agreement
I understand that dancing is a strenuous physical activity. To my knowledge, my child has no medical problems or injuries that would prevent participation in class. I understand that it is my responsibility to inform a physician of any medical issues and to obtain written consent for participation. I agree to inform the instructor(s) of any known health condition that may affect class participation.

Injury Agreement
I agree to release Dual Star Academy of Dance and its participants, or any instructor from liability related to accidents or injury. I also agree to communicate in the event of an injury and understand that physical clearance from a doctor may be required to return to participating in class.

Jazz The jazz program focuses on building strong technique, strength, and flexibility.
Ballet The ballet program teaches proper placement, turnout, technique and vocabulary in a disciplined environment and positive atmosphere.
Poms The poms program focuses on preparing dancers for high school, college, or professional dance teams.
Hip-Hop This is a fun and up-beat class that integrates many different styles of hip-hop including pop and lock, street style, and break dancing.
Tap The tap program concentrates on both rhythmic and classical tap methods.
